Diagnosis: Pericarditis and early repolarization
Discussion: Note the classic Pan ST segment elevation, highly suggestive of pericardial disease. This pt also has some J point elevation, suggestive of early repol, common in young African Americans. The EKG changes seen in pericarditis are actually the result of an "epicarditis." The inflamation of the epicardium causes an outward current during the ST segment which results in an elevation of the ST segment. Why inflamation does this I am not certain however it has been suggested that this epicardial tissue, because of the inflamation is unable to depolarize as completely as the surrounding tissue. The result is that during the ST segment this epicardium is less positive as compared to more deep tissue. This causes a net outward current, which then shows up on the EKG.
