Description An fourth heart sound due to forceful atrial ejection.
Type and Detection Heard over apex; while patient is at at supine or left semilateral position.
Findings on
Examination

 
Low pitch extra heart sound at late diastole or early systole. Increases with forced inspiration, exercise, elevation of legs, and with increased venous return.
